Clinical Interpretation: Hemarthrosis
Hemarthrosis is bleeding into a joint cavity. This synovial fluid analysis shows:
- Extremely elevated RBC (3.2 million/mcL): Normal is <2,000 - indicating significant bleeding into the joint
- Bloody appearance and red color: Confirms gross blood in the synovial fluid
- Elevated nucleated cells (2,570/mcL): Inflammatory response to blood in the joint
- High neutrophils (68%): Acute inflammatory reaction to the hemarthrosis
Follow-up Testing Completed (01/16/2026):
Extensive workup was performed to identify the cause of hemarthrosis:
- Factor V Leiden: Negative - View Report
- Antiphospholipid Panel (all 6 antibodies): Negative - View Report
- C1 Esterase Inhibitor (serum & functional): Normal - View Report
- Tryptase: Normal (5.9 ug/L) - rules out mast cell disorders
- ESR: 2 mm/hr - no systemic inflammation
- CRP: <1 mg/L - no systemic inflammation
Cause remains undetermined. Possible etiologies include:
- Trauma (may be unrecognized)
- Unidentified coagulopathy not detected by standard panels
- Pigmented villonodular synovitis (PVNS)
- Vascular malformation
Continued rheumatology and/or hematology follow-up recommended.
